Where to Apply for a U.S. Passport

U.S. citizens must apply in person if Primary evidence of U.S. citizenship includes any of the following: previously issued, undamaged U.S. Passport, certified birth certificate, naturalization certificate or certificate of citizenship. The U.S. Department of State has published

The U.S. Department of State has developed an online application for U.S. Passport (DS-11 form). U.S. passport fees are $135 for adults (renewal are $110) and $105 for minors; expedited services cost $160 (effective July 13, 2010).

For those who are renewing their U.S. passport, you must complete Form DS-82 application (available online) and may be mailed.

U.S. citizens who are applying for a passport for the first time may not complete the DS-11 form online. Other passport exceptions to this process include the following:
